Payments & Cancellations
Payment is required in advance of all services and all payments are non-refundable. We accept all major credit cards, as well as FSA and HSA. Clients must cancel all appointments at least 24 hours in advance, although more advanced notice is preferred. Full payment will be assessed for later cancellations.
Waiver of Liability and Informed Consent Release
I understand that the sessions I participate in with Movement Art, LLC d/b/a Movement Craft (“Movement Craft”) are provided for the purpose of improving health, wellness, and fitness and can incorporate exercises which may include a focus on any of the following: breathing and restorative body work, flexibility, strength, stress reduction, relief of muscular tension, mobility, balance, and/or the balancing/aligning of the body for proper posture. If I experience any type of pain, discomfort, or feel ill during this/these session(s), I will immediately inform Movement Craft and cease any physical activity; furthermore, I will not participate in a session with Movement Craft if I am experiencing any symptoms of unexpected pain or illness.

Is this a good idea? If so, I can't tell if the "Custom Fields" add-on is the right choice, or "Client Information." Which should we choose? Alos, we are debating if we should use a service like DocuSign to verify signatures. If we decide we need one, will we be able to incorporate it into this form input area?
