Payments & Cancellations

Payment is required in advance of all services and all payments are non-refundable.  We accept all major credit cards, as well as FSA and HSA.  Clients must cancel all appointments at least 24 hours in advance, although more advanced notice is preferred.  Full payment will be assessed for later cancellations. 

Waiver of Liability and Informed Consent Release

I understand that the sessions I participate in with Movement Art, LLC d/b/a Movement Craft (“Movement Craft”) are provided for the purpose of improving health, wellness, and fitness and can incorporate exercises which may include a focus on any of the following: breathing and restorative body work, flexibility, strength, stress reduction, relief of muscular tension, mobility, balance, and/or the balancing/aligning of the body for proper posture.  If I experience any type of pain, discomfort, or feel ill during this/these session(s), I will immediately inform Movement Craft and cease any physical activity; furthermore, I will not participate in a session with Movement Craft if I am experiencing any symptoms of unexpected pain or illness.



Is this a good idea?  If so, I can't tell if the "Custom Fields" add-on is the right choice, or "Client Information."  Which should we choose?  Alos, we are debating if we should use a service like DocuSign to verify signatures.  If we decide we need one, will we be able to incorporate it into this form input area?

    - Bookly Customer Information Add-on: https://support.booking-wp-plugin.com/hc/en-us/articles/360001696154-Custom-data-in-client-s-profile-Customer-Information-Add-on- allows you to extend a customer profile with additional data about the client which gives you the option to create various types of custom fields with different questions and collect the required info right in the Customers section in Bookly and all the information will only be available right in the Customers section in Bookly.     - Bookly Custom Fields Add-on: https://support.booking-wp-plugin.com/hc/en-us/articles/115003963514-Custom-Fields-add-on allows you to create the different types of questions which the clients would need to fill out before the booking confirmation and all these reservation details will be added to your Bookly Calendar and can also be added to the email notifications for both your staff members and customers.     However, the signature input isn't supported as well as integration with DocuSign isn't implemented yet. Perhaps as a workaround, the link to the DocuSign material can be included in one of the fields as a hyperlink.

    I am wondering if there is a best practice for all my clients complete intake forms online that will be attached to each client profile based on what service they do each time - so it will need to be client based as well as service based.  Does anyone have a good idea as to how this should be done?
